Advanced Life Support Encouraging CPR Guidelines Observation

Advanced Life Support (ALS) or Advanced Cardiac Life Support (ACLS) is a treatment algorithm followed by healthcare professionals when a situation arises requiring cardiopulmonary resuscitation (CPR) in cases of cardiac arrest, arrhythmias, stroke, or other related medical problems like poisoning. Advanced Life Support consists of CPR administration using oxygen administration via a bag or mask and chest compressions (see CPR Certification), which are now recommended at a 30:2 ratio of compressions to ventilations for adults, children, and infants to produce higher coronary and cerebral perfusion pressures. The electrical activity of the heart is monitored and depending on the type of cardiac arrhythmia, defibrillation is applied, and medications such as adrenaline (epinephrine), amiodarone, or atropine are administered. Saline or colloids may also be given intravenously to increase the circulating fluid volume in the body. End tracheal intubation may also be necessary to secure the airway in some cases, see Online CPR Certification.

While performing CPR, members of the Advanced Life Support team consider eight forms of potentially reversible causes of cardiac arrest, also known as “4H4T” as seen on the CPR Cetification Classes. The “H’s” are hypoxia (low blood oxygen), hypovolemia (low blood volume), hyper/hypokalemia (high/low blood potassium), hypothermia (body temperature well below normal), followed by the “T’s” which include tension pneumothorax (tear in the lung leading to collapsed lung and distortion of major arteries), tamponade (fluid or blood in the pericardium compressing the heart), toxic and/or therapeutic chemicals or medications, or thromboembolism (blood clot in the heart or lungs). Early intervention, early access, early CPR, early defibrillation, and early advanced life support save lives. This team of medical professionals consists of physicians from all disciplines, anesthesiologists, emergency medical technicians, and others trained in ALS designated to respond in a cardiac emergency.

Pediatric Advanced Life Support (PALS) is very much like ALS only it is designed to aid the pediatric healthcare provider in recognizing and managing critically ill infants and children at risk for cardiopulmonary arrest. There are medical emergencies specific to this age group, and the focus is placed on early recognition of distress as related to a specific medical diagnosis. ALS and PALS training is provided to healthcare professionals that work with adults and children, including physicians, physician assistants, nurses, nurse practitioners, paramedics, and respiratory therapists, across the country by the American Heart Association. Chiller Evaporators

The chillers evaporator is a heat exchanger that transfers heat from a process or air conditioning water circuit to the chillers cooler liquid refrigerant. Most air cooled chillers in the UK chiller hire market will be fitted with a shell and tube evaporator, a plate heat exchanger evaporator or a coil in tank evaporator. A shell and tube evaporator is used primarily on chilled water applications. When a chiller is fitted with this type of evaporator the chillers cool liquid refrigerant flows through tubes encased in a shell.

The process or air conditioning circuit water fills the shell flowing around the tubes. As heat is transferred from the water to the chillers refrigerant the gas boils inside the tubes and the resulting vapour is drawn into the chillers compressor.

Hot water will enter the shell at one end, chilled water leaving at the opposite end.
A Plate heat exchanger evaporator can be used for chilled water or fluid cooling applications. Stainless steel evaporators are especially suited in food and beverage applications such as batch cooling or potable water systems.

A coil in tank evaporator comprises a coil block, usually copper, with aluminium fins to provide an efficient heat transfer surface. The coil block is then submerged in a chilled water tank which acts as a thermal buffer for the refrigeration system. This type of coil is suited to hire chillers used on process cooling applications, typically reactor cooling systems in chemical cooling, petrochemical cooling & pharmaceutical cooling systems.

Air Cooled Chiller Condensers

The condenser is a major component of a water chiller. The condenser is a heat exchanger that rejects heat from the chillers refrigerant to air. Some chillers are water cooled. A Water cooled chiller gives up its heat into relatively cool water from a cooling tower, air-blast cooler or adiabatic cooler. A typical air cooled condenser fitted to an air cooled water chiller will use axial fans (propeller type) to draw outdoor air over a finned tube heat transfer surface (heat exchanger).

The temperature difference or “delta T” between the hot refrigerant gas that is flowing through the condenser and the cooler outdoor air induces heat transfer. The heat reduction of the chillers refrigerant vapour causes it to condense into liquid. The last part of the chiller condenser is called a sub-cooler. The chiller sub-cooler reduces the liquid gas temperature still further, under its condensing temperature. The air cooled chiller is best suited to chiller hire applications. Chiller hire projects require systems to be delivered and installed quickly with the minimum of fuss.

How To Solve The Problem Of Wet Packs in Steam Autoclaves and Sterilizers.

As a manufacturer of steam autoclaves, we at Medi-Clave frequently hear feedback from current and prospective customers alike, regarding condensate issues (wet loads) in their chamber, post-sterilization, impeding satisfactory processing results. In fact, it could very well be the single leading technical issue we have run into over the past 15+ years of manufacturing sterilizers! To understand why, you really have to understand the myriad of potential causes, to appreciate the complexity of solving this simple problem.

When helping our clients identify the source of the issue, we begin by evaluation of the most common suspects first. Foremost among those suspects is the steam supply. Frequently we find that steam quality is the culprit in leading to excessive condensate formation within the autoclave. Our first recommendation to clients is to determine if you are placing excessive demand on the boiler. Next, examine the steam circuit/supply and look for potential areas of condensate formation such as dead legs or improperly trapped or insulated piping. The duty of a steam trap is to discharge condensate while not allowing the escape of live steam. The further away the steam line is from the heating source/medium, the more likely condensate is to form, and require removal from the system. One should consider proper insulation for piping,the further away the steam line is, the more the need for insulation. Lastly, have your sterilizer service people check the jacket steam trap and check valve at the drain port to be sure they are clear and functioning properly.

If you or your staff have no experience with proper steam supply design/piping, we would recommend that you contact Autoclave experts or Steam Sterilizer expertsfor an evaluation.

If the water content of the steam is 5% by mass, then the steam is said to be 95% dry and has a dryness fraction of 0.95.satisfactory dryness fraction is .97) Once you have established that the steam supply is not the source (of excess condensate), it is time to look more closely at what is being loaded into the vessel, the design of the sterilizer itself and perhaps the operation of the autoclave.

Another common cause of wet loads is the load itself. However tempting it may be to pack the chamber full of product, in order to keep up with demand, (dense packing) is frequently a cause of excessive condensation which cannot then be completely flashed off by subsequent steam injections. Large quantities of hard goods or even complex packaging can make proper steam circulation a challenge.  

Steam enters the chamber and contacts the product, it is essential that the steam collapse (condense) on the product in-order for the heat to be released to the load. But ultimately the water formation must be discharged through condensate management or re- vaporized in order to prevent contamination of the product. Removal of the excess water is crucial to prevention of insulating the load from the steam. That said, too much steam, at too fast a rate can result in excessive water formation which might overwhelm or swamp steam traps. Be sure that your steam traps are properly sized.

To dry the load after the fact, many users are tempted to employ excessively long deep vacuums at the end of the cycle. Because the chamber is heated (via the jacket) and the fact that water will flash off into steam at a lower temperature under vacuum, this is relatively successful. But what ultimately results is an unnecessarily long cycle time, and potentially a non-sterile load as water can collect in areas, and insulate the intended product from achieving temperature. Deep long post-sterilization vacuum drying phases are like closing the barn door after the horses have escaped.

When we are asked to assist under these conditions, we typically look to prevent (or greatly reduce) the formation of condensate by making sure that the load itself is brought up to a certain temperature prior to the introduction of steam. One simple way this can be accomplished is by defining a proper pre conditioning phase at the beginning of the cycle.

In Summary: Check the quality of your Facility Steam, including possible excessive demands on your boiler.
Check piping insulation from your steam source, to the autoclave. Check the way you pack your loads and do not over pack the chamber! Check the size and operation of the steam-traps and the drain globe valves leaking through. Check for clogged drains pipes and drain ports. Check your vacuum drying phase program settings and operation. Check or add a controlled pre-conditioning phase at the beginning of the cycle.

As an alternative you might wish to consider simply increasing the jacket temperature set-point, as long as it does not interfere with maintaining the temperature uniformity within the chamber during the sterilize dwell phase. A word of caution, merely increasing the jacket temperature could result in the super-heating of the steam, in the case that your condensate issue is not particularly bad, so use this strategy cautiously.

There are some unique approaches that can be causes, or solutions, to the condensate formation issue.

These include: Charge Rate Control (CRC). Many autoclaves are not equipped with a proportional control valve, or the necessary control system capability, to control the ramp up or charge rate of steam injection during the cycle. If a chamber does not have some means of valve configuration, or software limitations to control the rate that steam is added to the chamber, the result could be a flooding of the chamber with steam that results in instant condensate formation (this is a common problem found in South African Hospitals and can be effectively rectified  by adapting to the right approach) . If the steam injection were to be ramped up more slowly, the amount of steam and subsequent condensate can be more easily controlled.

There are many causes for condensate formation, in fact, condensing of steam is essential in heat transfer and hence sterilization, but too much condensate is detrimental. If you are experiencing such problems, try some of the above in identifying the cause, or contact Medi-Clave for assistance.

Froth insulation spray offers another method of insulating a building. It is inexpensive in the energy output it saves you. It’s also greener than other insulating materials and flame resistant. Foam is effective as a sealant because it expands to fill every crevice and empty space, even between rafters and joints. In this manner, the insulator keeps out air and waste, among other stuff.

As part of a fire protection system, its sealing feature is also of huge advantage. With the foam, you don’t have to stress about air pockets in the material where combustibles can collect or cause other hazardous Problems. You also do not need to fear flames getting into unfilled pockets to either burn the structure beneath or become hotbeds should a fire happen. In addition, the undeniable fact that the insulation keeps air out stops more air from reaching a blaze or causing deadly backdrafts.

One of the best features about fire resistant insulation is that the material itself doesn’t burn. Even if fire somehow gets into the insulation, the substance doesn’t catch fire. Admittedly, after extended exposure to flames, the surface shows burning, but the flames never take hold. Considering the cost of rampant fire damage, charring is a much better and more reasonable trade off.

In trial tests, investigators built three sample crawl spaces. This trial was conducted to compare the performances of one kind of insulator to an untreated’calibration’ construction. This first was employed for a calibration test. The other 2 were treated with one sort of a spray in insulation product. Each crawl space was tested to a’failure time’ in which the flames consumed the plywood or couldn’t be sustained.

The calibration deck exhibited steady flames from the front after 6 mins of exposure to a flame source placed within the construct. After 9:30 minutes, the blaze penetrated the plywood.

The second and 3rd tests reflected similar results, which surpassed the outcome of commercial standard tests. Particularly, after 1:40 minutes, the controlled blaze ate into the insulation surface, but there was no ignition. After 3:40, there was sporadic ignition of the foam’s surface. Ignition ceased after 8:30 minutes. In almost 10 minutes of exposure, there was no burn through of the test material nor the plywood above it.

The investigators permitted the flame source to burn until it collapsed, a period of nearly 20 minutes. The test was finally ended after twenty-six minutes. There had been burning over the whole of the insulating surface, and some loss of material where the flames touched the surface, but the plywood was completely intact as was the majority of the spray in material.

Other products don’t last nearly as long in tests. Clearly, fire resistant insulation is an effective way to protect your building. The advantages are clear from the tests. Choosing a green, simple to install and flame resistant product would be of great benefit. With the properties of froth insulation spray, you can install it in a new or existing building. A sports car that has been flooded is likely to experience numerous difficulties that will shorten its lifespan.

Among the risks is the greater likelihood of significant rust problems. A previously soaked interior lends itself to rusting out floorboards and other often hidden spots.

More importantly, however, are the very real mechanical problems that can come about due to the flooding of a sports car. If water levels reached a high enough point, it is possible that grit, grime and water entered the engine and exhaust system. The residue can significantly impair performance and can shorten the car’s effective life considerably.

Flooded sports cars also frequently suffer from electrical problems. A soaked electrical system lends itself to corrosion and an increased risk of short circuits. Repairs will be necessary, and electrical work can bring with it considerable expense.

Fortunately, it is possible to avoid purchasing a sports car that has been flooded. There are a few things you can easily due to mitigate the risk of acquiring a previously flooded car.
